Submit a Claim - Claim Instructions

Class Members can choose to submit a claim to receive Settlement benefits.

If you submit a valid claim form, assuming the Court approves the Settlement, you will receive a Participating Class Member Payment. You will be bound by the Court’s judgement and release claims against Defendant relating to the Security Incident.

Claim Forms must be submitted online no later than January 14, 2025, or, if mailed, postmarked or received no later than January 14, 2025.


How to File Online:

After clicking the button below, you will be asked to provide the Unique ID you received on your Notice from the Claims Administrator. If you did not receive a Notice or have lost yours, please contact the Claims Administrator to obtain your Unique ID.

After submitting your completed Claim Form online, you will receive an email with a confirmation code for your completed submission. Receipt of a confirmation code means your Claim Form was successfully submitted. If additional information is required to complete your Claim, you will be contacted by the Claims Administrator.

How to File by Mail or Email:

If you wish to submit a Claim Form via standard mail, you may download a copy of the Claim Form here. You will need to provide all of the information requested on the Claim Form, sign it, and date it. Then, mail it to the following address:
